- Certificate of State of Origin
- Medical Fitness Certificate from a government hospital
- NYSC Discharge or Exemption Certificate (for graduate positions)
- National Identification Number (NIN) Slip

- Chief Superintendent of Quarantine Cadre (CSQ) – Veterinary Doctors (DVM)
- Superintendent of Quarantine Cadre (SQ) – Degree holders
- Deputy Superintendent of Quarantine (DSQ – Law) – Law graduates
- Deputy Superintendent of Quarantine (DSQ – Master’s Degree) – Master’s degree holders
- Assistant Superintendent of Quarantine II (ASQ II) – HND and Bachelor’s degree holders
- Inspector of Quarantine (QI) – NCE holders
- Inspector of Quarantine (QII) – OND holders
- Inspector of Quarantine (QIII) – OND holders
- Quarantine Assistant (QA) – SSCE holders
